Summary /n /n /n /nSpeech Therapist (PRN) – HomeCare MD/n/n Service
Area: Baltimore County/City & Carroll County, Maryland /nReports
to: Clinical/Therapy Manager /n/n HomeCare MD is seeking a
compassionate and skilled PRN Speech Therapist to join our growing
home care team. This role is ideal for an experienced Speech
Language Pathologist who is passionate about providing exceptional
patient-centered care in the comfort of a patient’s home.
/n/nPosition Summary/n/n The Speech Therapist is responsible for
conducting initial and ongoing patient assessments, delivering
skilled Speech Language Pathology services, and acting as case
manager for speech-therapy-only patients. This role will
collaborate closely with physicians, caregivers, and
interdisciplinary team members to support optimal patient outcomes.
/n/nEssential Functions/n/n /n/t - /n/t Develop individualized
plans of care tailored to each patient’s needs. /n/t /n/t - /n/t
Recommend and train patients on augmentative and alternative
communication methods (e.g., automated devices, sign language).
/n/t /n/t - /n/t Teach patients techniques to improve speech
production, voice quality, and oral/written language skills. /n/t
/n/t - /n/t Educate and train patients in safe swallowing
strategies or strengthening exercises to prevent aspiration. /n/t
/n/t - /n/t Maintain accurate and timely documentation, including
evaluations, progress notes, and reports for insurance
justification. /n/t /n/t - /n/t Conduct research-based assessments
and contribute to treatment methodologies. /n/t /n/t - /n/t
Participate in interdisciplinary assessments and collaborate in
creating and evaluating patient care plans. /n/t /n/t - /n/t Attend
case conferences and required agency meetings. /n/t /n/t - /n/t
Deliver services as prescribed by the patient’s physician and in
accordance with the plan of care. /n/t /n/t - /n/t Provide patient,
caregiver, and family counseling and education. /n/t /n/t - /n/t
Communicate effectively with physicians, healthcare practitioners,
and the QAPI department. /n/t /n/t - /n/t Participate in
agency-sponsored in-service training and work collaboratively with
clinical leadership. /n/t /n /n/nEducation & Experience
Requirements/n/n /n/t - /n/t Current Maryland Speech Language
Pathology License. /n/t /n/t - /n/t Master’s degree from an
accredited college or university. /n/t /n/t - /n/t Current American
Heart Association BLS for Healthcare Providers. /n/t /n/t - /n/t
2–3 years acute care experience preferred. /n/t /n/t - /n/t 2–3
years community health experience preferred. /n/t /n/t - /n/t Valid
driver’s license, automobile insurance, and ability to meet fleet
safety requirements (travel required). /n/t /n/t - /n/t Strong
knowledge of medical terminology. /n/t /n/t - /n/t Excellent
interpersonal, communication, and people management skills. /n/t
/n/t - /n/t Basic computer proficiency; Microsoft Office Suite
preferred. /n/t /n/t - /n/t Experience with electronic medical
records; Kinnser software experience preferred. /n/t /n/t - /n/t
Basic knowledge of Medicare/Medicaid/OASIS/Commercial Insurance
requirements. /n/t /n /n/nLeadership Skills/n/n /n/t - /n/t Ability
to motivate team members and adapt education to individual learning
needs. /n/t /n/t - /n/t Establishes positive, respectful
relationships with colleagues. /n/t /n/t - /n/t Capable of

Proficiency in EMR documentation and reporting. /n/t /n/t - /n/t
Ability to work closely with clinical and QAPI departments and
respond to requests in a timely manner. /n/t /n /n /n /n /n /n /n
