Required skills & experience (to be considered) - Bachelor's Science of Nursing required - Experience of 3-5 years preferred with knowledge in more than one discipline - Maryland Registered Nurse License or intent to achieve MD licensure if out of state required - Case management certification required within 3-5 years of hire for this role - Manages a team of professional staff, composed primarily of RN Care Coordinators, social workers and ED Community Health Workers - Responsible for providing care management services to the hospital's patient population that consists of care coordination, care progression, discharge planning, medical necessity reviews, and psycho-social services Perks Professional development opportunities Full benefits 401k Schedule details Day Care settings Acute Details Manager- Emergency Department Care Management Location: Baltimore, MD Job Category: Management & Supervisory Schedule: Full-time Shift: Day shift Job Description: Position Summary: Responsible for the oversight of daily operations, staffing, and workflows for the ED Care Management Department. In collaboration with the hospital-based leader, this professional manages a team of professional staff, composed primarily of RN Care Coordinators, social workers and ED Community Health Workers (CHW's). This leader is responsible for providing care management services to the hospital's patient population that consists of care coordination, care progression, discharge planning, medical necessity reviews, and psycho-social services. This leader also works in collaboration with the ED department, medical and nursing staff to manage ED length of stay, throughput, community resource referrals and the care of patients at risk for readmission. Under the Director's guidance, the Manager leads the team in continuous process improvement activities to ensure that patients receive cost-effective, high quality, and safe care. This professional also collaborates with both hospital and care management leadership to create and achieve department goals and initiatives. This includes supporting the tracking, trending, and reporting of department metrics that reflect the hospital's financial performance and progress toward patient satisfaction goals. As a member of the leadership team, the incumbent serves as a role model that supports professionalism, service, positive communication, teamwork and safety. Essential Functions: Assessment: Performs initial Care Management assessment to determine, based on patient's condition and presentation, care coordination and discharge planning needs as appropriate in the Emergency Department. Planning: Creates a focused, anticipated discharge plan of care for assessed high risk patients with identified needs. Intervention: Collaborates with the clinical team and medical provider to initiates the anticipated discharge plan for high risk patients in the emergency department, as appropriate. Performance Improvement: Stays abreast of changing organizational, emergency department, Care Management, and clinical trends, regulatory matters and third-party payer requirements related to clinical care, discharge planning, and precertification or aftercare benefits. Operations Management: Assists facility leadership with organizing, developing and coordinating the organization's EDCare Management Department and its activities according to the organization's needs while considering the patient's needs and payer requirements. Customer/Peer Relations: Works in a professional and collaborative manner with all staff, patients and families. Quality Assurance: Supports and promotes quality standards and expectations. Identifies and recommends opportunities for process improvement further promoting optimal patient and organizational outcomes. Provides focus on readmissions, denial prevention, length of stay Education Functions as advisor, resource and leader for staff and external department members. Viewed as the resident expert with regard to status determinations, care progression, and regulatory guidelines. Clinical Practice: Demonstrates clinical competence in the area of care management to include care progression, navigation, application of MCG criteria, and discharge planning. Leadership: Works as a collaborative member of the team to ensure quality of department services and in support of the departments agenda and goals. Requirements: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Bachelor's Science of Nursing required, experience of 3-5 years preferred. Maryland Registered Nurse License or intent to achieve MD licensure if out of state required., AHA CPR certification and BLS certification required. CCM required to obtain within 3 years of hire, Case management certification required within 3-5 years of hire for this role. Frequent technical/clinical skills in MCG, Curaspan, and Indisha knowledge, levels of post-acute care knowledge, Nursing home, ALFs, Home/Palliative/ Hospice care. Demonstrate knowledge of ED Diversion and Triage to appropriate alternate levels of care/community
