  The Nurse Transitions Navigator is a professional who has experience and expertise in navigating the health care system and providing assessments, education, and resources for high risk individuals.
  
  This individual works with the clinical team(s) to identify individuals who are at high risk for increased utilization.
  
  
  • 
    
  • The RN Transitions Navigator is directly involved in managing the multiple elements that comprise a person's successful transition from hospital to the community.
    
  • They will conduct patient visits, provide health education, assist in the coordination of health care needs, and develop professional referral relationships to ensure patient needs are met.
    
  • This individual will develop a plan that assists patients to close any gap in their transition to include the use of technology, addressing barriers to care, and supporting the patient's self-management of chronic illness based on clinical standards of care.
    
  • Manages Patient Transition:Manages the multiple elements that comprise a person's successful transition from hospital to home
    
  • Coordinates patient's health care needs and provides Health Education to Patient and Families
    
    
    Qualifications/Requirements:
    
    
    • 
      
    • Bachelors of Science in Nursing (BSN)
      
    • Working knowledge of theory and practice within a specialized field
      
    • Ideally, experience: 2+ years combined with Technical/Clinical Skills (Usage)
      
    • Maryland Registered Nurse License
      
    • American Heart Association CPR Certification
      
      
      Physical Demands:
      
      
      • 
        
      • Light - Exerts up to 20 lbs. of force occasionally, and/or up to 10 lbs. of force frequently to move objects.
        
      • Physical demands are in excess of those of sedentary work. Light work usually requires walking or standing to a significant degree.
