Baltimore and the nearby community. This includes the creation of
new healthcare jobs that will continue to improve how patients are
treated across care settings.

key role in providing high-quality care and improving patient
education and care for tens of thousands of patients. Preference is

The Social Worker will provide short term care coordination
services to individuals discharged from the hospital. The Social
Worker is responsible for supporting all aspects of a clients
health, social and environmental needs including: insurance
eligibility, navigation of healthcare services, linkages to
community resources, follow up appointments and the development and
monitoring of a care plan. The Social Worker will work in
collaboration with the hospital multidisciplinary team to ensure
goals and objectives are met.
Work is performed in a variety of community settings, (e.g.
hospitals, office, public agencies), or in the client s home. A
valid driver s license is required and extensive physical exertion
via walking, standing and/or climbing stairs may be required.
Bachelor s Degree required; Master s Degree in Social Work
preferred. Current social work licensure in the state of Maryland,
and a valid driver s license. CPR certification required. Minimum
of one (1) year of professional experience as a licensed social
worker. Requires experience in counseling and casework for the
terminally, chronically, and acutely ill adult population. Home
care experience preferred.
Requires a high level of interpersonal skills to effectively
interact with all levels of staff. Must have excellent written and
verbal communication skills. Requires ability to enter data into
electronic information systems and to interpret basic data
presentations and reports. Must be computer literate, specifically
MS Word, Excel, and Outlook.
