Job Description:

QUALIFICATIONS:1. Personal stability, flexibility, energy, and commitment to working with the chronically mentally ill.2. Current CPR and First Aid; driver's license and safe driving record.3. Bachelor's degree and/or two years of experience working with chronically mentally ill clients.4. Experience in teaching, social work, or vocational training is desired.5. Strong interpersonal skills to include verbal and written communication.6. Meet credentialing and privileging requirements as set out in Policy and Procedures.DUTIES:1. Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards, and Maryland State Regulations.2. Schedule appointments one month in advance with members using the EHR, input data, and documentation through to completion.3. Carrying out assignments to fulfill the organizations mission including but not limited to:Teach activities of daily living (ADLs) and independent living skills.Appointments and coordination of care with other providersRecreational activitiesTeaching basic standards of cleanliness & safetyMember assessment, personal problem solving and interventionsTraining in accessing community resourcesProvide Health Home servicesJob placement and supported employment counselingTeaches independent management of finances, medications, and housingTransportation4. Support member's healthcare needs and follow-up care.5. Regular visits of the members home to ensure safety and cleanliness maintaining agency standards and support member to achieve rehab goals to obtain or continue community living.6. Positive role modeling and ensure communication is respectful and culturally competent.7. Ability to take appropriate actions in an emergency and complete a Serious Incident Report when the situation is resolved.8. Learn the basics of authorizations and tracking.9. Participate in Health and Safety exercises by role modeling, practicing, and educating members with the member emergency drills and preparedness.10. Collaborate with the member in creating an IRP using person centered planning.11. Using the electronic medical records, responsible for collaborating with members on monthly progress notes, complete schedules, and contact notes.12. Extend trust and care while maintaining professional boundaries13. Take part in all agency-sponsored training programs as required by supervisor.14. Participate in team meetings.NARRATIVE: The team will be the center for information, documentation, decision-making, and implementation of plans for individual members in their residence and in the community. The team will be responsible for the goals and strategies that make up the core of an individual member's program and progress. The Psychiatric Rehabilitation Specialist will have shared responsibility for all members on the team.AUTHORITY: Position reports to the Team Leader. Schedules and additional duties subject to change in accordance with need of program and members; will be provided by the Team Leader.CLASSIFICATION; SALARY AND BENEFITS:Regular, full-time, exempt from overtime.Salary range - $36,000 - $47,500.PI153632024
