Location: 4940 Eastern Avenue, Baltimore, MD 21224
Shift:
Full Time (40 hours)
Day Shift; 8:00am - 5:00
Exempt; Pay Grade: CK
Position Summary:
In collaboration with patient/family, social workers, physicians
and interdisciplinary team, the Social Worker II provides direct
psychological, social and practical care including social work
assessments, problem identification, goal setting and
implementation of care plans. He / She Collaborates with all
disciplines to optimize patient outcomes; scope of services may
include, but is not limited to: developing action plans to mitigate
psycho-social barriers to discharge; patient/family engagement and
education; employing interventions (crisis intervention, grief
counseling, suspected abuse reporting) to address immediate
needs.
Education:
MSW required
Work Experience:
Minimum of two to five years (of which two years are health care
related field).
Must have expertise in one or more of the following areas:
