As a Telephone Case Manager RN with UnitedHealth Group, you'll support a diverse member population with education, advocacy and connections to the resources they need to feel better and get well.   • Build relationship with the member and the hospital staff
  • Collaborate with facility Case Management/Discharge Planning team to coordinate care and identify priority needs/members
  • Engage with facility Case Management/Discharge Planning teams and the member/family or caregiver to prepare for transitions and assist in facilitating the discharge plan
  • Enter timely and accurate discharge date and disposition of member into case management tool
  • Notify Clinical Programs (such as CTP. TTS, PAT, or House Call practitioners) of discharge disposition
  • Make outbound calls and receive inbound calls to assess members' current health status during the 30-day post discharge period
  • Act as a liaison between the facility and inpatient care manager to coordinate clinical information necessary to facilitate medical necessity determinations
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management
  • Make referrals to outside sources
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on WheelsThis is high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Years of post-high school education can be substituted/is equivalent to years of experience.Required Qualifications:
    • Current unrestricted Registered Nurse license in the state of MD and DC
    • 3+ years of experience in a hospital, acute-care or direct care setting
    • Data entry experience into case management systems
    • Demonstrate a proficiency in computer skills to navigate a windows-based environment, including internet searches, program navigation, and the use of multiple screens and tabs
    • Proven ability to be credentialed at assigned hospital and meet all hospital occupational health requirements (drug screening, licensure and immunizations)
    • Access to reliable transportation and ability to travel to hospital location for primary work sitePreferred Qualifications:
      • Case management experience or CCM certification
