Job Summary
Seeking a skilled Social Worker to provide comprehensive, family-oriented services to individuals in the home setting, assessing patients' and families' needs and formulating a plan of care.

Job Description
We are seeking a skilled Social Worker to join our team. As a Social Worker, you will provide professional, comprehensive, family-oriented services to individuals in the home setting. You will assess patients' and families' psychosocial, environmental, and financial needs, and formulate, implement, and evaluate a plan of care in collaboration with patient, family, and other caregivers. You will also provide case management as appropriate and offer counseling services to help patients and families cope with terminal illness.

Responsibilities
--- Assess patients' and families' psychosocial, environmental, and financial needs
--- Formulate, implement, and evaluate a plan of care in collaboration with patient, family, and other caregivers
--- Provide case management as appropriate
--- Offer counseling services to help patients and families cope with terminal illness
--- Coordinate with community and agency homecare referrals for services

Requirements
--- Master's Degree in Social Work from an accredited school
--- 2-4 years of clinical experience, with one year in a community health setting preferred
--- LCSW - Certified Social Worker
--- LCPC - Licensed Clinical Professional Counselor
--- Professional licensure required in Maryland and Pennsylvania
