CLINICAL SOCIAL WORKER (LCSW-C)JOB SUMMARYThe Clinical Social Worker provides comprehensive social work services for the patient population served and care management services to those patients in clinical programs.ESSENTIAL FUNCTIONS OF POSITIONPsychosocial assessment of familiesParticipation in the development of an interdisciplinary treatment planProvides educational, supportive and counseling services to familiesCoordination of services with community agenciesParticipating in discharge planning, and patient post discharge follow-up.In the care management role, plan, coordinate, implement, monitor and evaluate the care and resources availableAchieve high quality and cost effective patient outcomes across the continuum of careFunctions with a focus on patient safety, customer service, and family centered careParticipates in performance improvement initiatives for the departmentPOSITION REQUIREMENTSEducational RequirementsMasters Degree in Social Work from a University accredited by the Council on Social Work Education.Licensure, Certification and RegistrationWork Orientation and ExperienceKnowledge, Skills and AbilitiesOther Requirements Specific to Assigned AreaCurrent license to practice Social Work in the State of Maryland. LMSW Required, LCSW-C preferred. Case Management Certification as applicable required within one year of eligibility.Two years experience as a Social Worker in an acute or rehabilitation hospital with pediatric experience preferred. Experience in child welfare, family service, or mental health setting, is also desirable. Experience with organization and coordination of care for complex families; discharge planning, interagency coordination, case management and utilization management is preferred.Knowledge of normal growth and development for infants to young adultsKnowledge of family functioning and the nature of chronic illness/disability and its impact on the patient and familySkill in individual, family and group treatment modalities.Ability to effectively communicate both orally and in writing with interdisciplinary team members, family members, community agencies, referral sources and payers.Ability to work effectively with an economically and culturally diverse population.Ability to balance patient/family needs with constraints imposed by third-party payers and agency regulations.Knowledge of third party payer systems including Medicaid and Managed Care OrganizationsDetail orientedAbility to multi-taskExcellent communication skills and interpersonal skillsTact and sensitivityTime management and organization skillsAbility to effectively negotiate and build strong collaborative relationshipsAbility to walk, run, stand, sit, reach, stoop, bend, rush, pull, demonstrate manual dexterity and mobility and has visual and auditory ability to do this jobREPORTING RELATIONSHIPSSupervision ReceivedReports to the Director of Care Management/Social Work/Admissions.
