Job Title: Case Manager - Adult Medicine/Surgery Profession: RN

experience License: Active RN License Certifications: BLS
Must-Have: Current in-patient Case Management experience;
familiarity with EPIC and post-acute facility referral platforms
such as NaviHealth or AllScripts Description: The Case Manager is
responsible for coordinating care for a select caseload of
patients. They will manage care from triage through transition
planning. Coordination involves ensuring safe transfer to home or
appropriate post-acute settings. Proactive communication with
payors is essential to secure approval for treatment plans and
necessary resources. Maintaining knowledge of clinical systems and
fiscal data related to specific patient populations is crucial. The
Case Manager will collaborate with a multi-disciplinary team and
engage with patients and their families. Effective transition
planning is integral to this role. Responsibilities include
managing patients in accordance with departmental procedures while
prioritizing clinical safety and quality. The Case Manager will
enter and maintain patient information in all relevant departmental
systems. It is important for Bedside Case Managers to hold a
current CPR card, as they will not be involved in medication
