Job Description:
AccentCare, a leading home health care provider, is seeking a
compassionate and dedicated Hospice Nurse RN to join our team. The
role involves delivering personalized end-of-life care to patients
in their homes, ensuring comfort and quality of life during their
final days. This position requires a person who is skilled in pain
management, symptom control, and emotional support, tailored to
meet the unique needs of each patient and their families. As a
Hospice Nurse RN with AccentCare, you will play an essential role
in our mission to improve the well-being of our clients by
providing exceptional care and support. Assess patients’ medical
conditions and work with healthcare teams to develop care plans
Administer medications, monitor vital signs, and provide pain and
symptom management Educate patients and families on end-of-life
care and support Coordinate with physicians, social workers, and
other healthcare professionals to ensure comprehensive care
Document patient care services by charting in patient and
department records Provide emotional and psychological support to
patients and their families Strong knowledge of nursing practices
and principles Expertise in pain management and palliative care
High emotional intelligence and empathy Excellent communication and
interpersonal skills Ability to work independently and as part of a
multidisciplinary team Medical Insurance Dental Insurance Vision
Insurance 401k Current Registered Nurse (RN) license in the state
of employment Certified Hospice and Palliative Nurse (CHPN)
certification preferred
