Job Description:

in the D.C.-Maryland-Virginia region, is seeking a Clinical
Pharmacist II to join our dynamic team. Dedicated to delivering
exceptional patient care, we emphasize a patient-first approach in
all our services. This role requires an individual with a detailed
understanding of pharmacological methodologies and a commitment to
optimizing patient outcomes through innovative and integrated
healthcare solutions. Conduct medication therapy management and
review patient prescriptions to ensure correct therapy and dosage
Provide pharmaceutical care to patients, adhering to established
medication protocols and standards of care Collaborate with
healthcare providers to discuss patient treatment plans and
optimize medication regimens Assist in the training and mentoring
of pharmacy staff and students Engage in research and education
activities to remain current with the latest developments in
pharmacology and healthcare Ensure compliance with federal, state,
and local regulations concerning pharmacy services Administer
vaccines and participate in health outreach and education programs
Strong knowledge of pharmacotherapy and disease management
Proficient in using electronic health record (EHR) systems
Excellent communication and interpersonal skills Ability to work
collaboratively in a multidisciplinary healthcare team
Detail-oriented with strong problem-solving skills Medical

Active Pharmacist license in the state of practice Board
Certification in Pharmacotherapy (BCPS) preferred
