Job Description Job Description JOB SUMMARY The Home Care
Coordinator, Registered Nurse role offers a unique opportunity at a
new PACE (Program of All-Inclusive Care for the Elderly) program in
West Baltimore within the framework of a supportive, innovative and
expert leadership team and company. Together with our coalition of
strong partners within the community, the Home Care Coordinator,
Registered Nurse will work closely with the Clinic Manager, RN to
carry out our clinical model of care and have a transformative
impact on a particularly under-served community in West Baltimore.
The Home Care Coordinator, Registered Nurse is responsible for the
planning and monitoring of in-home services, hiring and ongoing
supervision of the Home Care Scheduler and Home Healthcare Aides
(HHAs). ESSENTIAL DUTIES AND RESPONSIBILITIES Serves as a required
member of the Interdisciplinary Team (IDT) and in initial,
semi-annual, unscheduled, and annual assessments; attend morning
updates and report changes in participants’ baseline status to
appropriate staff on a daily basis. Involved in the development and
implementation of QAPI activities. Ensure that the home care
schedule is accurate and that participants are allocated enough
time/services. Provide back-up for the scheduler when indicated.
Develop and update policies and procedures as they pertain to
in-home services. Plan for adequate staffing of in-home workers
Provides in-home care to participants. Responsible for supervision
of homecare personal care attendants and homecare LPN. Coordinate
service orders with contracted home care agencies, monitor delivery
of services and monitor quality of services provided by home care
contractors. Assure that documentation for home care is accurate,
current, and that the home care charts are up to date. Provide
follow-up for home care complaints as specified in thePACE of West
Baltimoreprogram grievance policies and procedures; document
complaints per PACE program grievance policy.

Provide on-call coverage during center hours of non-operation.
Maintain confidentiality regarding participant, staff, contractor,
and organizational information. Additional responsibilities will be
added at managers discretion, as career growth opportunities and

start-up environment. QUALIFICATIONS Minimum of two (2) years of
demonstrated successful experience in home care; prefer in-home
care management experience. Experience in the provision of nursing
care, includingassessmentskills, health education, and current
nursing treatments and protocols. Effective management skills.
Knowledge ofutilizationreview,qualityimprovementand cost
containment systems. Effective oral and written communication
skills. Knowledge of the principles and techniques involved in
relating to frail elderly personsrequired Mustsubmitto and pass a
pre-employment screening and criminal background check Must have a
current Driver’s License and driving record mustbe in compliance
withCompany policy. Mustmaintaincurrent automobile insurance. Must
have up to date vaccinations according to CMS/PACE of West
Baltimoreguidelines including vaccination against the seasonal flu
and Covid -19 Must have working knowledge of computers and
Microsoft Office applications. Operates in full compliance with all
applicable federal, state and local fire, health, safety,
sanitation, Emergency procedures and other standards prescribed in
