Responsibilities: Deliver primary care for short-term and long-term post-acute patients. Diagnose illnesses, manage chronic conditions, and develop individualized care plans. Order and interpret labs and diagnostics; prescribe and monitor treatments. Collaborate with healthcare teams and educate patients and families. Build strong, trusting relationships with patients, families, and facility staff. Requirements: Minimum 1 year of primary care experience (post-acute care a plus).
