Physical Therapist Location: Brooklyn Park, MD Agency: Ambition 24Hours Pay: $1,435 per week Shift Information: Rotating - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Occupational Therapist Skilled Nursing/Rehab Brooklyn Park, MD Role overview An Occupational Therapist is needed for a full time day shift contract at a skilled nursing facility in Brooklyn Park, MD (Hammonds Lane area), serving adult and geriatric residents in post acute rehab and long term care. This 14 week new opening runs standard weekday/day hours with a 35 hour weekly guarantee and is focused on functional recovery, ADLs, and safe discharge. Assignment details Location: SNF/rehab facility near 613 Hammonds Lane, Brooklyn Park, MD 21225 Setting: Skilled Nursing Facility (SNF) / rehab center Position type: Contract, full-time, non permanent Contract length: 14 weeks (02/23/26 05/25/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break (5x8 days) Days: Facility set day schedule (Mon Fri typical; weekend availability may be requested) Guaranteed hours: 35 hours per week Blended Gross Rate: $45 $55 Order reason: New opening Key responsibilities Evaluate residents ability to perform ADLs, functional mobility, and self care in a SNF rehab setting. Develop and implement individualized OT care plans to improve independence, safety, and quality of life. Provide interventions including ADL retraining, therapeutic exercise, neuromuscular re education, balance/coordination training, and adaptive equipment training. Collaborate with PT, SLP, nursing, and social work on interdisciplinary care plans and discharge goals. Accurately document evaluations, daily treatment notes, progress notes, and discharges in the EMR. Educate residents and caregivers on home programs, energy conservation techniques, and safe use of equipment. Required qualifications and skills Active Maryland Occupational Therapist license (or eligibility before start). Graduate of an accredited OT program. Recent SNF or geri rehab experience strongly preferred. Current BLS/CPR for healthcare providers. Strong documentation, time management, and interdisciplinary communication skills.
