Job Description:
MedStar Health seeks a dedicated CTO Transition of Care LPN Care
Manager to join our team, providing critical transitional care
management across our healthcare network. This role is central to
ensuring seamless transitions for patients across care settings,
especially post-discharge, to reduce readmissions and improve
health outcomes. As a proactive member of the healthcare team, you
will coordinate with physicians, nurses, and other healthcare
professionals to implement care plans that reflect the
organization's patient-first philosophy. This role requires a
strong clinical foundation, excellent communication skills, and a
passion for patient advocacy. Effectively communicate with
patients, families, and all healthcare professionals to ensure a
clear and consistent care transition process. Initiate and update
personalized care plans in collaboration with healthcare teams,
tailoring interventions to patient-specific needs. Monitor patient
progress post-discharge, promptly identifying and addressing any
barriers to recovery. Educate patients and families about
self-management strategies, including medication adherence and
follow-up care instructions. Lead and participate in
interdisciplinary team meetings to assess patient progress and
coordinate care. Document all patient interactions, decisions, and
interventions in accordance with HIPAA regulations and
organizational policies. Strong clinical knowledge and skill set
Ability to work effectively in a dynamic and changing environment
Exceptional interpersonal and communication skills Experience with
electronic health record (EHR) systems Knowledge of healthcare
regulations and accreditation standards Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Must possess current
LPN/LVN licensure. BLS Certification required.
