Using advanced technology and your clinical expertise, you’ll assess, plan, and manage individualized care that promotes healing and independence. You will be responsible for case managing a team of 15-20 hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.
