Job Description:

Licensed Practical Nurse (LPN), you will provide compassionate and
efficient care, ensuring the wellbeing of patients in their homes.
Your role is pivotal in providing the daily care and emotional
support these patients need while adhering to their personalized
care plans. This position offers a rewarding opportunity to work
autonomously and as part of a dedicated healthcare team committed
to high standards of health and patient care. Administering
medications and treatments as prescribed by the healthcare
provider. Monitoring patient health condition, recording signs of
progress, and reporting any sudden changes to the necessary
professionals. Assisting with daily needs such as dressing, bathing
and hygiene to ensure patient comfort and support. Educating
patients and their families on proper home health care strategies
and procedures. Coordinating with physicians and other healthcare
professionals to create and execute personalized care plans.
Proficiency in administering medications and managing IVs Strong
observational skills to detect changes in patient conditions
Effective communication skills for interacting with patients,
families, and healthcare teams Ability to work independently and
manage time efficiently Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Valid LPN license in the state of
practice CPR certified Driver’s license and reliable
transportation
