Assess patient and family psychosocial needs and develop care plans accordingly. Provide end-of-life counseling and grief support to patients and their families. Coordinate with healthcare teams to ensure holistic care delivery. Assist families with navigating healthcare and financial systems. Document patient interactions and changes in condition adequately. Strong empathetic communication skills Proficient in crisis management Effective coordination and organization skills In-depth knowledge of health care policies and hospice care regulations Medical Insurance Dental Insurance Vision Insurance 401k Licensed Clinical Social Worker (LCSW) Certified Hospice and Palliative Social Worker (CHP-SW) preferred
