Job DescriptionUniversity of Maryland, Baltimore is currently seeking an Advanced Practice Pharmacist for the School of Pharmacy, Pharmacy Practice and Science. The selected candidate provides clinical services at the professional level focusing on medication use in patients and populations. The candidate will be responsible for training pharmacy students and residents. Participate in medical rounds five days per week unless the staff is "off-service" and completing other tasks such as projects and/or teaching. This staff member will round in the medical intensive care unit six months per year and on the acute heart failure service three months per year. The remaining time will be spent covering leave for faculty and staff and/or completion of projects and teaching. Changes to drug treatment are recommended in addition to educating medical team on proper use of medications, side effects, monitoring parameters, and assessing response. Actively monitors medication use in individual patients and populations on a daily basis. Reviews, analyzes and interprets patient information including health records, medical forms, laboratory results and records of past medication use.Precepts pharmacy students and pharmacy residents.

ESSENTIAL FUNCTIONS\:

  • Participate in medical rounds. Changes to drug treatment are recommended in addition to educating medical team on proper use of medications, side effects, monitoring parameters, and assessing response. Monitors medication use in individual patients and populations on a daily basis. Reviews, analyzes and interprets patient information including health records, medical forms, laboratory results and records of past medication use.
  • Precepts pharmacy students and pharmacy residents.
  • Actively participates in teaching and scholarship activities including publications on manuscripts and poster presentations at national pharmacy meetings
  • Provides direct patient care through attendance of patient care rounds with medical intensive care unit and acute heart failure teams at University of Maryland Medical Center. The pharmacist may provide coverage for other adult, inpatient services.
  • Reviews, analyzes and interprets patient information.
  • Provides recommendations about appropriate medication therapy for patients in the hospital.
  • Reviews patient profiles for appropriateness of medication therapies.
  • Documents consultations and prescriber interactions, creates reports that summarize clinical activity and presents them to supervisors and reviewers.
  • Provide patient counseling.
  • Provide quality improvement services necessary for accreditation, certification or maintenance of permits and licenses.
  • May perform coordination of care services for your patients.
  • Precept students nine months per year in the hospital. Precept residents.
  • Financial initiative\: will participate in cost containment and resource utilization projects and will document cost reductions in a manner requested by pharmacy leadership.
  • Research initiative\: conducts and participates in research by disseminating original research through publication in peer review literature and/or presentation of written or delivered reviews on current topics at local, regional and national meetings.
  • Committee service for school and hospital including Pharmacy & Therapeutics Committee as assigned.
  • Participate in clinical pathway or guideline development and maintenance when appropriate and/or when requested in order to maintain best practices within the institution.
  • May dispense and distribute medication.
  • Opportunities for teaching laboratory sessions and/or didactic lectures.Education\: Bachelor degree in pharmacy required. Doctoral degree in pharmacy (PharmD.) preferred.

    Experience\: Two (2) years residency specializing in Cardiology, Critical Care or Pharmacotherapy. Residency or practice experience managing the following adult populations\: medical intensive care unit, mechanical circulatory support, heart transplant, advanced heart failure and pulmonary hypertension.

    Certification\: Board Certified Pharmacotherapy Specialist is preferred.

    Licensure\: Current pharmacist licensure in good standing that will be eligible for reciprocity with State of Maryland Board of Pharmacy. Able to secure and retain an active State of Maryland Pharmacist license for employment;

    KNOWLEDGE, SKILLS, AND ABILITIES\:
    • Comprehensive knowledge of pharmaceuticals, their indications, interactions and potential side effects as it pertains patient who are critically ill.
    • Knowledge and understanding of relevant physiology, pathophysiology and disease states.
    • Skill in the assessment of patients and their drug regimens, the development of pharmacotherapeutic plans, in teaching health professional students and residents, in communicating with people of all levels, in use of relevant PC applications including MS Word and Excel and relevant library databases including Medline .
    • Excellent organization and time management skills.
    • Knowledge of relevant accreditation and certification requirements.
    • Comprehensive knowledge of pharmaceuticals, their indications, interactions and potential side effects. Already noted above.
    • Knowledge and understanding of relevant physiology, pathophysiology and disease states.
    • Skill in the assessment of patients and their drug regimens.
    • Skill in the development of pharmacotherapeutic plans.
