We Are Hiring Mostly day from afternoon to evening. May be rotating. Safe Streets Hospital Response Program, overseeing Violence Interrupter Program Violence Interrupter experience What You Will Do Responsibilities: Planning, coordination and directing activities of the Safe Streets Hospital Responder Program, including supporting the work of the Hospital CaseManagers and the Hospital Responders. Implementing trauma-informed and culturally relevant policies, programming, and training in coordination with Safe Streets Baltimore, the Baltimore City Health Department, and Cure Violence. Providing a leadership role in maintaining communication with hospital staff and the program staff to coordinate effective implementation of program activities. Coordinating rapid response of hospital responders to hospitals following notification of the admission of a victim of violent injury and the development and follow-up on the long-term treatment plan for each participant in the program. Participating in hiring, strategic planning, trainings, and evaluation activities to support the sustainability of the program and integration into the central hospital response to violent injury. Working closely with other City and State agencies and local hospital systems to implement program policies and procedures that readmission rates of victims of violent injury. Meets with interested patients and loved ones following a violent injury for follow-up assistance during their recovery from their injuries Assesses patients and visitors for follow-up needs and facilitates linkage to appropriate supportive services Acts as participant advocate, coordinates and makes referrals for services. Monitors and tracks services provided and must document all participant encounters and contracts made on behalf of participants. Coordinates and monitors services, including comprehensive tracking of participant activities in relation to action plan. Collaborates with other staff to formulate action plans and strategies to help resolve conflicts and prevent re-injury. Maintains records and participant related files, writes reports, composes letters and completes forms, as appropriate. Participates in regular staff meetings conducted by Safe Streets Baltimore and the hospital. What You Will Need Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Work Experience: 3 years of experience required. 5 years of experience preferred. 1 year of leadership or management experience preferred. Additional Preferences At least one year of experience with managing staff and/or have strong experience providing case management to at-risk populations. Must be able to work with victims of violence and guide victims to needed resources and services. Must have experience using data systems and basic computer skills. Experience working with returning citizens or other populations disconnected from mainstream society.
