Part-Time

9:00AM - 1:00PM

Monday-Friday

Requirements:

• Establishe effective system for information retrieval for patient opportunities and data.
• Utilizes computer programs to create and manage database files.
• Collects data and prepares monthly departmental reports.
• Reconciles data in Epic through data investigation and documentation.
• Completes regular outreach to schedule patients for visits at ambulatory care practices.
• Coordinates projects related to contacting patients for quality initiatives, contacting patient documenting procedures and outcomes.
• Providing feedback and monthly reporting to Quality and Transformation team.
  Qualifications:
  
  High School Diploma and Some College. Degree preferred.
  
  EPIC, OUTLOOK and Basic Microsoft Word
  
  1-2years in Healthcare preferred
  
  Communication/Attention to detail/Customer service
