required; experience in an acute care medical setting. Description:
The Medical Social Worker II (LCSW-C) is responsible for
implementing the plan of care for specific patients. This process
begins upon admission and follows through until discharge.
Collaboration with the patient, family, interdisciplinary care team
members, and external healthcare providers to ensure safe, timely,
and efficient care is essential. The social worker initiates the
plan of care and monitors its implementation. The role functions as
part of a team coordinating care for patients and serves as a
problem-solving resource. Innovative skills for continuous
improvement in patient outcomes and cost-effectiveness are
required. Internal communication outside of the workgroup is
necessary to persuade or gain cooperation and acceptance of ideas.
