About this Job: General Summary of Position MedStar Health is
seeking a Master’s-level Social Worker (MSW) to join our Homecare
team. In this role, you'll provide critical social work services to
both home health patients and clients under a Montgomery County
contract. You’ll offer counseling, connect patients with community
resources, and collaborate with healthcare team members to address
psychosocial factors affecting treatment, rehabilitation, and

Provide psychosocial assessments and develop individualized care
plans with the patient, family, and healthcare team. Deliver
short-term counseling and facilitate access to community resources
as needed. Support patients with discharge planning, nursing home
placements, and resolving social barriers to care. Maintain timely,
accurate documentation in compliance with federal/state regulations
and agency policies. Participate in interdisciplinary meetings,
committees, and community outreach on behalf of the department.
Foster strong working relationships with internal teams and
external partners. Stay current through ongoing education and
training. Qualifications: Master’s degree in Social Work (MSW) from
a Council on Social Work Education (CSWE)-accredited program.
Active licensure required upon hire: LGSW, LCSW, LCSW-C, LISW, or
LICSW. Minimum 2 years of experience in a healthcare setting. Valid
driver’s license and reliable personal vehicle required. Current
CPR, BLS, and NRP certifications from the American Heart
Association or American Red Cross. Home care experience preferred.
This position has a hiring range of : USD $61,838.00 - USD
$111,259.00 /Yr.
