Job Description:
MedStar Health, a leading non-profit healthcare organization in
the DMV area, is seeking an RN Case Manager dedicated to excellence
and patient-first care. This role is pivotal in managing patient
care coordination in various settings across our network. The RN
Case Manager assesses, plans, implements, coordinates, monitors,
and evaluates the options and services to meet an individual's
health needs through communication and resource management. This
involves collaborating with patients, families, healthcare
providers, and insurance companies to ensure effective and
efficient patient care delivery. Conduct comprehensive patient
assessments to identify health needs and goals. Coordinate and
implement care plans with healthcare teams to ensure quality and
cost-effective outcomes. Regularly evaluate patient progress and
revise care plans as necessary. Facilitate communication between
patients, family members, and healthcare providers. Manage
resources efficiently to minimize unnecessary healthcare
expenditures. Ensure compliance with healthcare regulations and
standards. Strong clinical skills in a healthcare setting Excellent
communication and interpersonal skills Proficient in healthcare
management systems Ability to work collaboratively in a
multidisciplinary team Strong organizational and time-management
skills Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Current Registered Nurse (RN) license in the state
of Maryland; Certification in Case Management (CCM) preferred.
